This Consumer Disclosure Notice explains our limits of liability. There are two copies; sign and return the Agency Copy, and retain the Client Copy for your records.

While we highly recommend the purchase of travel insurance for your protection, we realize that not everyone will purchase it. If you are not purchasing Travel Insurance from Carefree Vacations, please complete and return this Travel Insurance Waiver form to us. We are unable to release your travel documents without the purchase of travel insurance, or this signed waiver.

Traveling with children under age 18, and without both parents? Get these forms completed and notarized, and make sure you have them with you when you travel!

We are happy to provide these forms as a service. If you fail to have them completed and notarized, the child may not be able to travel with you! You do not need to return these forms to us.

If you are traveling with a minor that you do not have legal custody of, please have the legal guardians complete this Minor Guardian Appointment form.

If only one parent of a minor is traveling outside the United States with the minor, please have the other parent complete this Minor Travel Authorization form.
